Анотація

Актуальність дослідження зумовлена необхідністю проведення розширеного пошуку інформації щодо сучасного стану проблеми гострого середнього отиту у дорослих на основі досвіду вітчизняних та іноземних авторів.
Мета дослідження: провести аналіз світового досвіду по проблемі етіології, патогенезу та діагностики гострого середнього отиту у дорослих.
Матеріали та методи дослідження: 125 сучасних літературних джерел електронних баз даних РubMed, EMBASE, MEDLINE, U.S. National Library of Medicine Clinical Trials, Research Gate та Cochrane Library
Вступ: Проблема середнього отиту як складова проблеми запальних захворювань верхніх дихальних шляхів залишається актуальною в сучасній медицині, виходячи з показників його захворюваності і поширеності в структурі отоларингологічних хвороб. Незважаючи на досягнення сучасної медичної науки, означені показники залишаються стабільно суттєвими, що зумовлює актуальність подальших досліджень з метою удосконалення діагностики та лікування.
Результати: проведенням бібліографічного методу дослідження встановлено, що більшість наукових робіт присвячена проблемі гострого середнього отиту у дітей на тлі приділення недостатньої уваги даній проблемі у дорослого населення. Клінічними і дослідженнями в експерименті встановлено, що функція імунного захисту середнього вуха притаманна практично всім фізіологічним рівням від епітеліальних клітин до внутрішньоклітинного ядерного фактору. Означена функція реалізується через месенджери цитокінової мережі. Серед пулу цитокінів важливе значення для патогенезу гострого середнього отиту набувають ІЛ-1, ІЛ-6 та ФНП-α. Особливістю ефективності імунного захисту від патогенів є доведена в
експерименті влучна в часі зміна стадій запалення із притаманними для кожної з них процесами. Показано, що кожна з ланок імунного захисту (включаючи цитокіни) здатна виявляти (в разі невідповідності в силі та в часі) негативний вплив з підтриманням або посиленням запалення. Наведені дані експериментального аналізу патоморфологічних змін в середньому вусі в залежності від терміну хвороби. Показано також, що питання ускладненого перебігу (зокрема, мастоїдиту) все ще залишається актуальним. Встановлено роль гострого середнього отиту в розвитку внутрішньочерепних ускладнень (менінгіту). Доведено значення коморбідності для патогенезу гострого середнього отиту та роль цукрового діабету в структурі коморбідності. Переконливо окреслені основні чинники зниження ефективності діагностики та лікування
гострого середнього отиту: мультифакторний характер етіології, особливості імунного захисту і схильність отопатогенів до ухилення від імунної відповіді хазяїна, а також значення супутньої патології у дорослих пацієнтів із означеною ЛОР-патологією. Певна кількість літературних джерел присвячена особливостям перебігу гострого середнього отиту на тлі цукрового діабету.
За результатами аналізу літературних джерел виявлено недолік: недостатня кількість досліджень присвячених особливостям клініки та діагностики гострого середнього отиту у дорослих, особливо на тлі коморбідної патології, а саме – цукрового діабету 2 типу свідчить про актуальність подальших досліджень по означеній проблемі.
